| Since ISC is the examination conducted by ICSE board you have to mark it ICSE. |

| You have to complete the application form yourself. Incase any part of application form is incomplete, it is liable to get rejected by computer, the responsibility shall lie with the candidate. Your status of acceptance / rejection of application can be seen on this website. |

| My application form has been rejected. How do I get it corrected? |
| You can not make corrections to the old form. You can resubmit another application after purchasing a fresh application form from the designated branches of
bank(s). |

| I have made mistake / overwriting / smudging of the application form. What do I do now? |
| This is a computer readable form; hence it should not be smudged / folded. Overwriting is also not allowed. Use of any eraser or correcting ink whitener is also not allowed. Computer will not be able to read it and hence reject it. Incase you have made any such error please purchase a new form and fill it afresh. You are advised to make a photocopy of the form and practice it out first before filling the actual form. |

| Will my form be rejected? |
| Your form is likely to get rejected if you have not filled any of the required fields, made any overwriting / smudging / folding of the OMR sheets which makes it non readable by computer. Be advised that the OMR sheet shall be read by computer and any such carelessness can lead to a nonreadable status and rejection. You should read the instructions given in the brochure carefully, make a photocopy of the OMR
and practice out first before filling the actual form.
Incase of any mistake there is no option but to purchase
a new form. Also your photograph should be as prescribed format, a face size of less than 20cm is quite likely to get rejected as well. |

| Why has my application form been rejected? |
Your form will be rejected if it is incomplete in any aspect including:
1. Name of Candidate
2. Father’s (Guardian’s) name
3. Mailing Address.
4. Photograph. [Should be as per prescribed format. A face size of less than 28cm will get rejected]
5. Signatures.
6. Index finger impression.
7. Age.
8. Mutilated application forms, or application form or barcode has been tampered with.
9. Duplicate or forged or photocopied application forms. The examining body shall not check your eligibility. Eligibility issues will be decided by the counseling board at the time of counseling and/or the College/University where you get admission. |

| We have insured that the forms shall be available at the designated branches of banks; detailed list is given in the advertisement and also on this web site. In case it is not available in your area; you should check the nearest branch of the bank from this website. | The OBC and General Category forms are the same. The SC/ ST forms are available separately |

| The eligibility criteria are clearly given in the information brochure. The examination body is NOT going to check your eligibility criteria. At the time of counseling the counseling body shall determine the eligibility of the merit listed candidates and if found ineligible at the time of counseling you shall not be allowed to participate. Hence in the interest of candidate it is advisable to go through the eligibility status thoroughly before applying. |

| Do I have a domicile of UP? |
A. You have to get the domicile certificate as per the prescribed format given in the brochure
You will have to ascertain your eligibility yourself. The Examining body will not ascertain your eligibility or advice you on the same. Eligibility issues will be decided by the counseling board at the time of counseling and/or the College/University where you get admission. Your application form will not be rejected because of eligibility issues. Hence appearance in the CPMT examination does not guarantee that you are eligible. |

| Only domicile of UP are eligible and have to submit certificate as prescribed in the application brochure. |

| The new pincode system of our country is having 8 digits code, the last two digits are the extension number of your area. If you don't know your extension number then you just fill up the old six digit pincode and leave the two extra boxes blank |

If you had Biology as an integrated subject in your +2 examination then fill up the Biology marks in the Biology box and leave the Zoology / Botany blank.
If you had Zoology / Botany in the +2 examination then you fill up the boxes for that and leave the Biology box empty. |

| Please refer to brochure carefully. You need 50% in the qualifying examination. If you have 50% in Intermediate then you do not need the BSc marks since you qualify as per Intermediate marks. If you don't have 50% in Intermediate but have 50% in BSc then you qualify through BSc examination and have to fill up to BSc marks boxes. |

| I have placed thumb impression at the place of index finger and dispatched the form. What should I do? |
| The form is computer readable and computer cannot decide the correctness of the impression. The Index finger impression shall be used as identity parameter. In case of mismatch with the impression of index finger at the examination center and counseling center with verification sheet impressions, a candidate shall not be allowed to appear and may be charged for forging. Such candidate should purchase a fresh / second form, fill it correctly and send to the university in time. They should also send a written request to the Chairman CPMT-2010 giving the reference/photocopy of previously submitted form and get the first/incorrect application form cancelled. |

| I have sent two application forms to the University. What should I do ? |
| As per the instructions given in the information brochure only one form of a candidate is to be submitted/accepted. Candidates having sent two application forms should immediately to Chairman CPMT-2010 giving the detail/photocopy of the form and get one of the form rejected/cancelled. Otherwise both the forms of such candidate shall be rejected and they may be penalized for forging. |

| The date shown on the photograph in entry no 15 of the sample OMR form is only for the purpose of example and has no relation with entry no 9. The date on your photograph should be written on the pattern shown in photograph (entry no 15) but it should be as per the instruction no 18 of the page no 2 in information brochure. |
